Short answer
The state and the county in which the notarization actually occurred — the physical location of the signer and notary at the moment of the act.

If a preprinted venue names a different county or state, the notary corrects it by lining through the incorrect entry, writing the correct one, and initialing the change — or by attaching a loose certificate with the correct venue if the state permits.
Example
Preprinted venue says "County of Orange," but the signing happens in Seminole County. The notary corrects the county to Seminole.
