The signer refuses to complete part of the document.
Short answer
A notary may not notarize a document containing blank spaces in most states. If the signer will not complete it, stop and refer them to the preparer.
Full explanation
Blanks invite later alteration, which is why so many states prohibit notarizing incomplete documents. It is not the notary's role to fill blanks or advise on content — the preparer or the receiving agency handles that.
Common mistake to avoid
Never write in a blank on a signer's document, even when they ask you to.
