Separate two issues: physical ability and mental awareness. Physical limitations have lawful accommodations — signature by mark, or signature by direction where authorized. Awareness has no workaround. Talk directly with the patient, ask open questions, and proceed only if they clearly understand and want to sign.

The patient can speak clearly and explain the document but cannot form letters. The notary uses the state's mark procedure with two witnesses and records it in the journal.
Common mistake to avoid
Do not guide the pen. Do not let a family member sign "for" the patient unless your state expressly authorizes signature by direction and you follow that procedure.
